Delivering best practice in the real world: The hypertensive patient

In this interactive case presentation, we will describe the elements of best practice at key points in a patient’s journey through the diagnosis and management of raised blood pressure. We will describe the decisions that the patient will face. We will ask the delegates for their input. The background will be a ‘shared decision making’ format where communication, in a manner that the patient understands, is key. Diagnosis, accurate measurement of blood pressure and best practice in initiating treatment will be covered. Follow-up, blood pressure targets, other risk factors and planning for the future will be included in a practical manner.
