Falls cost the NHS 2.3 billion pounds per year, 4 million bed days and 70 000 hip fractures. A significant number of these falls are caused by transient loss of conscience, which, in turn may be caused by syncope. Of course, syncope is not a disease, but a symptom of an underlying condition. In many cases there is a benign cause such as vasovagal syncope, whereas other cases have a more sinister aetiology. Dr Thakkar describes a systematic approach to managing syncope and the “3P’s”, how to risk stratify patients and how to avoid medico-legal pitfalls.
