Optimising Best Practice in Kidney Care

Clinical update & case study


This will be a very interactive and practical session, suitable for all members of the primary health care team.
The presentation will start with a clinical case based on a patient from the speakers practice. Unfortunately this will demonstrate sub optimal care and delegates will be asked to identify options for improvement.
The speaker will then use evidence from the National CKD Audit to demonstrate current practice in England and Wales and also results of the ASSIST CKD project showing a new model of care.
We will then return to the clinical case and optimise his care in the hope of a better clinical outcome.
We hope also to be joined by a representative from the British Kidney Patient Association to remind us of the impact of poor care on patient outcomes.
